Portal User Agreement:
Consent to Electronic Delivery. You specifically agree to be sent any and all "Electronic
Communications" (defined below) pertaining to all of your FLI investments via the FLI Client
Portal rather than through the U.S. mail. The term "Electronic Communications" includes all
FLI statements, tax forms and related disclosures, including Form ADV and updates to FLI's
privacy policies and procedures, and any and all current and future notices and/or
disclosures that various federal and/or state laws or regulations require that we provide to
you, as well as such other documents, statements, data, records and any other
communications regarding your FLI relationship. You agree that if you previously directed us to send your FLI communications to a third-party advisor or family member, such advisor or family member will be granted access to your FLI Client Portal.
You acknowledge that, for your records, you are able to use the FLI Client Portal to retain
Electronic Communications by printing and/or downloading and saving the Electronic
Communications. You accept Electronic Communications provided via the FLI Client Portal as
reasonable and proper notice, for the purpose of any and all laws, rules, and regulations,
and agree that such electronic form fully satisfies any requirement that such
communications be provided to you in writing or in a form that you may keep.
You acknowledge that certain of the documents to be delivered electronically will contain
information regarding your personal financial matters and consent to the delivery of such
personal financial information via electronic means. You further acknowledge that e-mail
messages are not secure and may contain computer viruses or other defects, may not be
accurately replicated on other systems, or may be intercepted, deleted or interfered with
without the knowledge of the sender or the intended recipient; and that FLI may intercept,
monitor and retain e-mail messages to and from its systems to the extent permitted by
applicable law. FLI retains the right to deliver some or all of your communications through
the U.S. mail rather than electronically.
Your consent is effective until revocation by you. You have the right to revoke your consent
to receive/obtain communications via the FLI Client Portal at any time. If you wish to
withdraw your consent please contact us at 516-935- 1200. Please contact us to report any
problems with electronic delivery. You agree to keep your email address current and notify
FLI of any changes. You will be notified by email when a document eligible for electronic
delivery is available for download.
